It's definitely done in the US, though "nightcrawler" usually refers to especially big and fat earthworms, the kind often used as fishing bait.

I don't think they are only in the US but the ones we have in my area can be HUGE - some of them they look like small snakes to me.

A night club usually has a stage show (singer or dancers), dinner and perhaps some dancing. A discoteque is strictly for drinking and dancing and, until the 1970s "Disco Craze", was mostly an "undergound" venue of questionable repute.

Can't speak for elsewhere, but if I asked someone in the UK if they wanted to come to a night club and we turned up to a cabaret act and dinner they would be very confused. Perhaps if I hopped in a time machine and asked someone in the 1930s... :)
