I disagree with Doctor Death. Joker was the main villain in Batman #1. He is obviously a supervillain, erego he is the first.

Ah, but Detective Comics #27 is the first Batman comic, not Batman #1. Dr. Death wasn't in that one, but he was in a later Detective Comics issue, before Batman #1.
