still not clear to me - the question mark means it's an Italian city but not a salami one? or that the hint is in some way wrong? (eg it could be a non-Italian salami city)

Genoa is a brand of American style salami, which many people incorrectly think comes from the Italian city of Genoa. The question mark is a common cryptic crossword clue device, which indicates that a clue is not to be read or understood literally. The "lion's meow?" clue is the same: lions don't actually meow, but the question mark indicates that this is not to be taken literally.
