While I totally agree that Bielefeld doesn't actually exists, Rancho Cucamonga is somewhere between Los Angeles and San Bernardino (touching two other cities present in the quiz).

When I was young, comedians on TV sometimes made fun of Cucamonga (without adding the Rancho) as a play on "Kookamunga". As in, "That lady was so crazy she must have come from Cucamonga."

Can't think of a good reason why San Juan in Puerto Rico is excluded from this quiz. What country is it part of, if not the United States? It would be like excluding Darwin in Northern Territory and Canberra in the Australian Capital Territory from a quiz about Australia, because they don't have statehood. Australian's don't exclude our territories in general thinking but Americans have really strong prejudices towards excluding theirs.

I would say that the Australian Capital Territory isn't a great analogy, as DC is included in this quiz.

A good analogy would probably be including Nuuk in a quiz about Denmark.

"Americans have really strong prejudices toward excluding" their territories. You've apparently drawn this thesis from the exclusion of San Juan on an internet quiz?? Geez.

(And for what it's worth, the equivalent in Australia wouldn't be Canberra because the quiz includes Washington DC, but it would be as if you left out a sizable city on Christmas Island or Norfolk Island, which are external territories)

It's simply the way people pull data. The Census Bureau lists all statistics for the country and then separately for territories. The entire constitutional requirement for a census is for congressional apportionment. Territories are not an integral part of the country. They can and do leave the control of the U.S. The states cannot. (Remember the Civil War?) Puerto Rico has time and time and time again had the opportunity to become a state with full representation or to become an independent nation. They choose instead to be U.S. citizens who don't pay the heavy federal income taxes regular U.S. citizens do, and harbor businesses which get away income tax free as well. I'd gladly save all that money every year not to have a useless congressman. No campaign ads to listen to.

So you put Washington DC in as Washington, with DC not as an acceptable type-in. As someone from the DC area with family living in the city itself, we've often referred to the city as DC. Almost no one I know calls it "Washington" without saying "DC" along with it.
