They still have masculine and feminine version, i think in all Slavic countries except ex Yugoslavia but including Macedonia.

Best if you just forget Cherie - probably the majority of Brits are trying to. Although it wasn't her fault, when some of the press started calling her First Lady it was so irritating.

For Nelson Mandela, you could argue that 2 of his 3 wives were quite famous. Winnie of course (wife number 2) but also his 3rd wife Graca Machel, who is his widow and also the widow of Mozambique's first president (and believed to be the only woman who was 1st lady of 2 different countries). Also the same can be said about the wives of Juan Peron, where 2 were (are) very famous. Eva Peron (wife number 2) is the most well known (outside of Argentina), but 3rd wife Maria Estela (isabel) was his VP and succeeded him as president upon his death.

Both the list itself and the results kind of point out the extent to which the U.S. has never gotten over certain monarchical aspects, such as the queen-like status of the "First Lady." As a politically-conscious Canadian, I have no idea what the names of any spouses of Prime Minsters have been since Margaret Trudeau (which takes us back more than three decades) - yet I have no trouble remembering the names of American queens.
