The instructions could stand some elaboration. For example, the Kentucky exclave pictured in the quiz photo can be reached from the rest of Kentucky by traveling on the Mississippi River without entering the land of Tennessee or Missouri, so, in some sense, it is "separated from the rest of the state by a lake or river".

Why is Minnesota excluded? Lake Of The Woods Northwest Angle is the most prominent exclave on the U.S. map.

there's only a lake between Minnesota and the northwest angle, and I'm not counting those as separations.
