'Highest' geography

Do you know which countries, states, regions & geographical things are the 'highest'? For some of these things, 'highest' can be in different senses.
Because sometimes it is difficult to measure elevation, some answers might be disputed or at the very least educated estimations.
Unless otherwise specified, elevation is height above sea level.
