Tone poem or symphonic poem would be more descriptive for Sibelius’s work.

It may be so, but if you know that the work is a tone / symphonic poem, you could definitely think of its name even without the specification.

Finlandia is not a poem. It is a musical, not a literary, composition. Yes, those "in the know' will get it, but it could be misleading for others.
