Previous home cities of current NBA teams

Given the name of current NBA teams, can you name the last city in which they used to be based? source: http://en.wikipedia.org/wiki/List_of_relocated_National_Basketball_Association_teams
Answers are where the team name has remained the same however the home city has moved
Where a team has moved multiple times, name only the most recent city
If the teams name includes/included a state rather than a city, then that is the answer. Not the city in which the team named after a state played
As of the start of the 2014/2015 season

It's as if the Fort Wayne team knew it would be going to Detroit.

Why did San Francisco pop up when I typed in Philadelphia? Last time I checked they were very different cities.

According to Wikipedia the San Francisco Warriors were named after the San Francisco Bay Area rather than the city itself, and since Oakland is in the Bay Area they technically haven't moved since becoming the Golden State Warriors. However before they were the San Francisco Warriors they were the Philadelphia Warriors, which is why I am accepting both answers
