About the question concerning the "Name of the document that was signed". The point is that this was not called the "Declaration of Independence. What they actually signed was (and is) headed "In Congress, July 4, 1776. The Unanimous Declaration of the thirteen united States of America". See: https://www.archives.gov/founding-docs/declaration

In other words, I think you might want to rethink that question, especially as the rest of it is such a nice quiz.
