Teams That Have Played in All Four Divisions of English Football

The Football League Fourth Division was introduced for the 1958/59 season. A total of 36 teams have competed in each of the top four tiers of English football, with a small number of them playing in the old First Division only before the creation of the fourth tier. How many can you name?
Two clubs require their full names to be typed out as they are both located in the same city. For example, "Shrewsbury Town" instead of just "Shrewsbury".
