This song is often reported to be about Cathy Smith, an occasional backup singer, drug dealer, and legal secretary who did 15 months in prison for injecting John Belushi with a fatal dose of heroin and cocaine in 1982. She had a relationship with this fellow Canadian. It was a violent relationship that was detailed in his divorce papers.
