Every New Football League Club Since WWII

Can you name all 36 teams that have joined the English Football League (EFL) for the first time in the years since the end of the Second World War?
Three of these teams were formed as "phoenix clubs", with another being set up in protest. The team that "joined" the League in 2004 may be a bit of a contentious one. One of the three phoenix clubs is Welsh. See comments for one piece of additional information.
Not all of these clubs currently compete in the Football League, with some having been dissolved and then reformed.
